Oshdoc Inc, a Delaware corporation ("Oshdoc," "we," "us," or "our") respects your privacy and is committed to protecting it through its compliance with this Privacy Policy.

Oshdoc is affiliated with the Oshdoc Medical Group, P.C., a California professional medical corporation ("Oshdoc Medical"). Oshdoc provides management and administrative services to Oshdoc Medical and Oshdoc Medical provides medical services, including clinical consulting services. 

This Privacy Policy describes the types of Personal Information Oshdoc may collect from you or that you may provide when you visit oshdoc.com (our "Website") and our practices for collecting, using, maintaining, protecting, and disclosing your Personal Information.

  • "Personal Information" means information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer or household, including Sensitive Personal Information.

    • Personal Information does not include Protected Health Information (as defined by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA")).

  • "Sensitive Personal Information" means Personal Information that reveals a consumer's (A) social security, driver's license, state identification card, or passport number, (B) account log-in, financial account, debit card, or credit card number in combination with any required security or access code, password, or credentials allowing access to an account, (C) precise geolocation, (D) racial or ethnic origin, citizenship or immigration status, religious or philosophical beliefs, or union membership, (E) mail, email, and text messages content unless we are the intended recipient of the communication, (F) Personal Information collected and analyzed concerning a consumer's health, (G) Personal Information collected and analyzed concerning a consumer's sex life or sexual orientation.

Children Under the Age of 18

Our Website is not intended for children under 18 years of age. No one under age 18 may provide any Personal Information to or on the Website. We do not knowingly collect Personal Information from children under 18. If you are under 18, do not use or provide any Personal Information on this Website or through any of its features, register on the Website, or provide any Personal Information about yourself to us, including your name, address, telephone number, email address, or any screen name or user name you may use. If we learn we have collected or received Personal Information from a child under 18 without verification of parental consent, we will delete that Personal Information. Personal Information We Collect About You and How We Collect It

Our collection of your Personal Information is governed by the terms of this Privacy Policy.

In the preceding 12 months, we collected the following types of Personal Information about users of our Website:

  • Identifiers. This includes Personal Information such as your name (real and/or alias), mail address, IP address, email address, account name or other similar identifiers.

  • Information that Identifies/Relates To/Describes/or Can Be Associated with You. This includes Personal Information such as your name, signature, work address, telephone number, education, employment, employment history, financial information, membership in professional organizations, or professional licenses and certifications.

  • Protected Characteristics. This includes Personal Information such as your national origin, citizenship, or sex (including gender, gender identity, gender expression).

  • Internet or other similar network activity. This includes Personal Information from your activity on our websites, mobile apps, or other digital systems, such as internet browsing history, search history, system usage, or electronic communications with us.

  • Geolocation Data. This includes Personal Information that discloses your physical location or general location.

  • Professional or Employment-Related Information. This includes Personal Information such as where you work or what job you perform.

  • Inferences. This includes Personal Information drawn from any of the above categories to create a profile about you, which reflects your preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities, and aptitudes.

  • Sensitive Personal Information. This includes Personal Information such as provider numbers, log-in and payment information, precise geolocation, citizenship, and your communications with patients. Oshdoc does not use or disclose Sensitive Personal Information for purposes other than those specified in Cal. Code Regs. Tit. 11 Section 7027(m).

We collect this Personal Information:

  • Directly from you when you provide it to us. This includes Personal Information entered into our Website at the time of registration or other continued use and records and copies of correspondence between you and Oshdoc.

  • Automatically as you navigate through the site. Information collected automatically may include usage details, IP addresses, and information collected through cookies or web beacons.

    • Cookies (or browser cookies). A cookie is a small file placed on the hard drive of your computer. You may refuse to accept browser cookies by activating the appropriate setting on your browser. However, if you select this setting, you may be unable to access certain parts of our Website. Unless you have adjusted your browser setting so that it will refuse cookies, our system will issue cookies when you direct your browser to our Website.

    • Web Beacons. Pages of our Website may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit Oshdoc, for example, to count users who have visited those pages or and for other related website statistics (for example, verifying system and server integrity).

Why We Collect Your Personal Information

We use Personal Information that we collect about you for the following purposes:

  • To present our Website and its contents to you.

  • To provide you with information, products, or services that you request from us.

  • To fulfill any other purpose for which you provide it.

  • To carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collection.

  • To notify you about changes to our Website or any products or services we offer or provide though it.

  • To allow you to participate in interactive features on our Website.

  • In any other way we may describe when you provide the information.

  • To a buyer or other successor in the event of a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of Oshdoc's assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which Personal Information held by Oshdoc about our Website users is among the assets transferred.

  • For any other purpose disclosed by us when you provide the Personal Information.

We may also disclose your Personal Information:

Does Oshdoc Sell or Share My Personal Information?

No, Oshdoc does not Sell or Share Personal Information. Additionally, Oshdoc does not have actual knowledge that it Sells or Shares the Personal Information of consumers under sixteen (16) years of age.

"Selling" means selling, renting, releasing, disclosing, disseminating, making available, transferring, or otherwise communicating your Personal Information to a third party in exchange for money or other compensation.

However, Selling does not include:

  • Disclosures or interactions with third parties as directed by you.

  • Transfers to processors that process Personal Information on our behalf.

  • Disclosures necessary for providing Oshdoc's services that you request.

  • Transfers to our affiliates.

  • Disclosures of Personal Information made publicly available by you.

  • Transfers as part of a merger, acquisition, or similar transaction, provided Personal Information is used consistently with the original terms.

"Sharing" means selling, renting, releasing, disclosing, disseminating, making available, transferring, or otherwise communicating a consumer's Personal Information to a third party for Cross-Context Behavioral Advertising, even if no money is exchanged. "Cross-Context Behavioral Advertising" means the targeting of advertising to you based on your Personal Information obtained from your activity across businesses, distinctly branded internet websites, applications, or services, other than such activity on Oshdoc's platforms.

However, Sharing does not include:

  • Disclosures or interactions with third parties as directed by you.

  • Alerting other third-parties that you have opted out of sharing your Personal Information.

  • Transfers as part of a merger, acquisition, or similar transaction, provided Personal Information is used consistently with the original terms.

Your Privacy Rights

Right to Delete

You may request that Oshdoc delete the Personal Information we collected about you. If your request is accepted, Oshdoc will delete your Personal Information from our records, and we will notify any service providers, contractors, or other parties with whom we have disclosed your Personal Information to do the same.

We may retain a confidential record of your deletion request to prevent the Personal Information from being sold or to comply with any applicable laws.

Right to Correct

You may request that Oshdoc correct inaccurate Personal Information. We will use commercially reasonable methods to comply with your request.

Right to Know What Personal Information is Being Collected/Right to Access Personal Information

You may request that Oshdoc disclose (1) the categories of Personal Information we collect about you, (2) the sources from which we collect the Personal Information, (3) the business or commercial purpose for collecting Personal Information, (4) the categories of third parties to whom we disclose Personal Information and (5) the specific pieces of Personal Information we have collected about you.

Right to Non-Discrimination

Oshdoc respects your right to privacy. We will not discriminate against you because you have exercised any of your rights under this Privacy Policy. Specifically, we will not deny you access to our services, charge you different prices, or provide a different level of quality of services if you exercise your privacy rights.

How Can I Exercise My Privacy Rights?

To exercise any of these rights you can send your request to privacy@oshdoc.com.

Upon receiving your request, we will need to verify your identity. These verification efforts require us to ask you to provide information so that we can match it with information you have previously provided us. For instance, depending on the type of request you submit, we may ask you to provide certain information so that we can match the information you provide with the information we already have on file, or we may contact you through a communication method (e.g., phone or email) that you have previously provided to us. We may also use other verification methods as the circumstances dictate.

We will only use Personal Information provided in your request to verify your identity or authority to make the request. To the extent possible, we will avoid requesting additional information from you for the purposes of verification. However, if we cannot verify your identity from the information already maintained by us, we may request that you provide additional information for the purposes of verifying your identity and for security or fraud-prevention purposes. We will delete such additionally provided information as soon as we finish verifying you.

An authorized agent, such as a parent, guardian, or conservator, may submit your request on your behalf, however, we will have to similarly verify the authorized agent's authority to submit such request.

We will respond to your request within 45 days, or we will notify you if we need to extend our review period for an additional 45 days.

Data Security

We have implemented measures designed to secure your Personal Information from accidental loss and from unauthorized access, use, alteration, and disclosure.

The safety and security of your Personal Information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Website, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.

Unfortunately, the transmission of Personal Information via the internet is not completely secure. Although we do our best to protect your Personal Information, we cannot guarantee the security of your Personal Information transmitted to our Website.
